In January 2000, LifeWise began offering a 3-Tier Pharmacy Benefit to some groups.
The Preferred Drug List (PDL) applies only to members who have the 3-Tier pharmacy benefit.

The LifeWise Prescription Drug 3-Tier Benefit Plan is designed to provide you with a high quality benefit-one that helps manage costs while still offering you choice.
The program is simple. With this benefit, your drugs fall into one of three categories or "tiers." Each tier has a different copay:
All generic drugs
Lowest copay
Preferred brand name drugs
Higher copay
Non-preferred brand name drugs
Highest copay
Sarah's copays under this program are $5/15/30 for drugs purchased at a retail pharmacy.
- If Sarah receives a generic drug, she pays $5 copay.
- If Sarah receives a preferred brand drug, she pays $15.
- If Sarah receives a non-preferred brand name drug, she pays $30.

- Ask your practitioner to prescribe a generic drug when appropriate. All generic drugs are covered and have the lowest copayment.
- If a generic drug is not appropriate, ask your practitioner to prescribe a preferred brand name drug on the Preferred Drug List (PDL). (These drugs have the Tier 2 copay)
- Ask your pharmacist to fill your prescriptions with a generic drug when allowed.
