All forms are available in both Microsoft® Word and PDF formats. You can fill out the Word documents online, print and sign; or print either format, and then fill out and sign.

Required for Campers
The Camper Registration and Camper Health forms are required for each camper. One Camper Release form is required for each camper per camp session.

Cascade Camp Cedarbrook offers camp shcolarships through our Joyce Hadeen Scholorship Fund. If you wish to request a scholarship, please fill out the Scholarship Request Form.

Cascade Camp Cedarbrook and Camp Koinona are partnering to make improvements on the camp property. We are currently working to build a pool and wash house. If you wish to support this project by donating to our Service Book, please fill out the Service Book Donation Form.
