States West Life offers life insurance, dependent life, short-term disability (STD) and long-term disability (LTD) for employers with 2 - 24 employees. Since the "true group" products begin at 10 employees, the small group program is used most commonly for groups of 2 - 9 employees.
States West Life does not charge a monthly billing or administrative fee. Minimum monthly premium is only $15 total for a combined package of Small Group Life/AD&D, Dependent Life and Short-term Disability. Long-term Disability requires a separate premium.

Small Group Dependent Life
The available option is:
  • Spouse ($2000), Children less than 6 months ($100), Children 6 months to age 19 ($1000)
  • Dependent life rate is $1.00 per dependent unit
  • Group Life must be selected for this benefit to be offered

Small Group Short-Term Disability
Available plan designs are:
  • Flat Amount ($100 - $400 maximum weekly benefit not to exceed 66 2/3% of Basic Weekly Earnings)
  • Percent of Earnings (60% of basic weekly income to maximum of $400 benefit)
  • Graded Benefits by class of employee


  • Plan 1: Officers/Partners/Owners $ 400
      Managers/Supervisors $ 250
      All Other Employees $ 100
    Plan 2: Officers/Partners/Owners $ 250
      Managers/Supervisors $ 150

  • Maximum Benefit Period of 13 or 26 weeks
  • Benefits are payable on the first day of total disability for an accident or eighth day for a sickness

Small Group Long-Term Disability
Available as a stand-alone product or packaged with other Small Group products, Small Group Long-Term Disability offers the following features:
  • 50% and 60% of Pre-Disability Earnings benefit
  • Standard monthly maximum benefit is $6,000 (Limited & Restricted Industries are limited to $3,000)
  • Elimination Period of 30, 60, 90 or 180 days (Limited & Restricted Industries are limited to 90 or 180)
  • Maximum Benefit Period – To Age 65 (RBD), 2 year/RBD, 3 year/RBD, 5 year/RBD
  • Definition of Disability includes 24-month regular occupation and any gainful occupation thereafter
  • 12/6/24 Pre-Existing Condition Exclusion
  • LTD coverage is guaranteed issue at 6 employees
  • Coverage available to restricted and limited industries allow States West Life to offer LTD coverage to nearly every employer
  • LTD coverage can be offered as a stand-alone benefit and in carve-out settings
  • No monthly billing or administrative charge and minimum monthly premium is only $50
