States West Life offers true group term life insurance, dependent life, supplemental life, voluntary life, short-term disability (STD) and long-term disability (LTD). True Group Term Life/AD&D--Voluntary Life Plan
Flexible plan designs options available:
  • Flat life benefit amounts
  • Percent of earnings
  • Graded benefits by class of employee
  • The Life maximum benefit amount may be up to 5 times the employee’s annual earnings. All plan features—like Accelerated Benefits for the terminally ill, Waiver of Premium for those who become disabled, and our Insured Benefit Account—add value to life for our insureds and their beneficiaries
  • The AD&D maximum coverage can be 3 times the Life amount. Our group AD&D, offered with our group life plans in amounts up to three times the life benefit, is essential to any group life program. The Seat Belt Rider pays an additional benefit to the insured’s beneficiary.
  • Evidence of Insurability Requirements are determined by the size of the group and plan design selected. Other factors taken into consideration are industry, age reduction, average benefit amount and age of employees.
  • Other features are Waiver of Premium, Conversion Provision, and Accelerated Benefits (50% to $100,000)
  • Dependent Life is available for spouse and children (up to 50% of employee benefit in some states)
  • Supplemental Life is available if written as part of basic Life contract (with minimum of 10 eligible employees). Up to $20,000 available on a Guarantee Issue if participation level is met. Supplemental life is simple, low cost, and the convenience of payroll deduction for premium payment keeps coverage in place.
  • Voluntary Life is available if written as part of the basic Life contract (with a minimum of 10 eligible employees). Aggressive guarantee issue limits and liberal participation requirements make the States West Life voluntary life plan one of the best. A variety of plan designs include a small group version for a few as 5 enrolled employees.
  • Voluntary AD&D is available with a variety of plan features. Voluntary AD&D is a valuable low cost way to add real benefit muscle to any employee benefit program. This plan does require basic life coverage and a minimum enrollment of 10 employees.

True Group Short-Term Disability
Short-term disability benefits can be purchased on a free-standing basis or packaged with life and LTD:
  • Benefit Percentages available are 50%, 60%, 66 2/3%, 70% of Weekly Pre-Disability Earnings
  • Weekly benefit maximum is $750 (standard). Potential max. is $2,500 (subject to approval)
  • Elimination Period – variety of available options
  • Maximum benefit period is 13 weeks up to two years
  • Core/Buy-up plans are available for STD programs
  • STD coverage can be contributory on non-contributory

True Group Long-Term Disability
LTD plans can be purchased on a free-standing basis or packaged with life and STD plan:
  • Benefit Percentages available are 50%, 60%, 66 2/3%, 70% of Monthly Pre-Disability Earnings
  • Monthly benefit maximum is $6,000 (standard). Potential max. is $30,000 (subject to approval)
  • Elimination Period - 30, 60, 90, 120, 150, 180, 360 days
  • Maximum benefit period options include 2 years and SSNRA, for example
  • Coverage can be contributory or non-contributory
  • With the optional Activities of Daily Living (ADL) definition of disability, you no longer have to be concerned with the percentage of blue-collar or high-risk occupations within the employer’s work force. By using the ADL features in our LTD plans, we can extend coverage to almost any industry and occupational mix. The ADL feature also offers significant premium savings. This allows LTD to be included in nearly any employee benefit plan.
  • Voluntary LTD is available with a minimum participation of 10 lives or 25% of eligible employees, whichever is greater
  • Core/Buy-up plans are available to better address employer budget constraints and still meet the employees’ demand significant for benefit levels

  • Integrates certain features of the LTD contract to the STD benefits such as workplace modification, partial disability and return to work incentive
